Make a referral

We walk alongside our families from diagnosis, through treatment and beyond.

Our Family Services team are currently running at capacity. We want to ensure that families get the best care and support from our outreach team; therefore, we have now paused any new referrals. This will allow us to maintain our high standards of support.

 

You can still refer families for a break at our Hampshire facility, The Bluebells – no outreach support included at this stage.

We offer an outreach service of emotional, social and practical support to families based in the South East, principally from Berkshire, Buckinghamshire, Hampshire, Surrey and Oxfordshire.

In order for families to be able to access our services, we need a completed referral form.

Details of our referral process can be found below.

Our eligibility criteria

Our criteria for taking on new referrals are that the child must be aged between 0-19 years old and has been diagnosed with a life-limiting condition according to NICE Guidelines:

Group 1 – Life threatening conditions for which curative treatment may be feasible but can fail. (E.g. cancer, irreversible organ failures of heart, liver, kidneys.)

Group 2 – Conditions where premature death is inevitable. (E.g. cystic fibrosis)

Group 3 – Progressive conditions without curative treatment options. (E.g. Batten disease, mucopolysaccaridoses, muscular dystrophy.)

Group 4 – Irreversible but non-progressive conditions causing severe disability leading to susceptibility to health complications and likelihood of premature death. (e.g. multiple disabilities such as brain or spinal cord insult.)

Self Referral

Step 1: Submit a referral to referrals@sebastiansactiontrust.org. You will need to give us consent to contact your medical professional. A confirmation e-mail will be sent to you to confirm the receipt of referral. The form can be found here.

Step 2: Medical Professional contacted. If the criteria are met, then you will receive an email to confirm that you will be contacted by your allocated family support worker. The assigned family support worker will make contact within 7 working days of receipt of the referral to arrange an initial visit and assessment at your earliest convenience.

If the criteria for you to receive outreach support from the Trust are not met, an email will be sent to you to confirm this and you will be signposted to other services that may be more suitable for your needs.

Medical Professional Referral

Step 1: Please fill in a medical professional referral form. As part of your referral you will be required to confirm that the child has a life-threatening or life-limiting condition in accordance with the NICE guidelines which are specified on the referral form. Additionally you will have also obtained consent from the family that they would like to access our service. The form can be accessed here.

Step 2:  Submit referral to referrals@sebastiansactiontrust.org A confirmation e-mail will be sent to you to confirm that the family will be contacted by their allocated family support worker. The assigned family support worker will make contact within 7 working days of receipt of the referral to arrange an initial visit and assessment at the family’s earliest convenience.

Professional Referral

Step 1: Please fill in a professional referral form. As part of your referral you will be required to provide details of a medical professional involved with the family so that they can confirm the child has a life-threatening or life-limiting condition in accordance with the NICE guidelines which are specified on the referral form. Additionally, you will have also obtained consent from the family that they would like to access our service. The form can be accessed here.

Step 2:  Submit referral to referrals@sebastiansactiontrust.org A confirmation e-mail will be sent to you to confirm that the medical professional will be contacted.

Step 3: Medical Professional contacted. If the criteria is met, then you will be emailed to confirm that the family will be contacted by their allocated family support worker. The assigned family support worker will make contact within 7 working days of receipt of the referral to arrange an initial visit and assessment at family’s earliest convenience. If the criteria is not met, an email will be sent to you to confirm this.

Out of County referrals

For families who do not live in Berkshire, Buckinghamshire, Hampshire, Oxfordshire and Surrey, but live in the UK, we can offer access to a respite break at The Bluebells. Subject to the referred child meeting our eligibility criteria.
